I officially give my permission, too.

I looked back at the original threads of Lori's, and she specifically stated that "When the quilt is done it will be donated." That should cover our butts, no?

We hereby give my permission for the quilt bearing the photo of Wardell Barker to be displayed as a donated gift to the Alzheimer's Foundation's quilt tour.

The latest E-Mail from Josie, Joyce, I see she CC'd a coy to you, and last I heard, you had signed & sent that form in, so this may be a repeat. Just wanted to keep you all informed, as much as I am, anyway!

Ladies,

I hope this email finds you both well. I just wanted to bring you up to speed on a couple of things:

1) Amanda Edmonds - our Quilt Director at the Alzheimer's Foundation of America is on maternity leave. She gave birth to
a beautiful baby girl a month ago. She'll be on leave until September. During her leave, Melissa Austen is taking over the
Quilt to Remember duties as well as overseeing our biggest initiative, National Memory Screening Day. Needless to say, her
hands are quite full! She is juggling between her email mausten@alzfdn.org and Amanda's aedmonds@alzfdn.org.
Melissa has been with the AFA for several months and is a tremendous asset to the organization.

2) The Forget Me Not Quilt panels are all completed - and will be posted on the website shortly. We are hosting an
educational conference in Chicago on September 24th and at that time I would like to display the Forget Me Not panel
collection with the rest of the panels but in order to do that we need some information.

Can you please be so kind as to assist Melissa with whatever questions she has in order to facilitate getting the panels
included in the Chicago tour date. While we understand it may not be possible to get release forms from everyone, we
need a letter stating that they are giving YOU permission to US the panels for display.

If you can get this taken care of by September 1st, I'd really appreciate it.

If you have questions, please call Melissa at 866-232-8484.

Thanks much.

Josie

I have sent the concent form to Amanda and told her I'd copy and send the permissions I receive here. She sounded like the one I sent was sufficient but guess it won't hurt to send everything.
The last two panels are complete and Melissa will be sending me a picture of them that I will relay here.

I don't see any problem with everything being in order for the showing.

Thanks for taking care of all of this Joyce. Can't wait to see the finished product.
