Hi all, Quilt peeps! I just received this E-Mail from Melissa, and apparently she needs "ME" to sign some consent forms, or release forms, probably so we never ask to be paid, even if they make money with them, and frankly, since there are 30 Loved Ones represented, and I would have a hell of a time, finding them all, I do not feel that I have the authority to sign these forms, but I do not want this to prevent them, from going on display? I just thought I would post this here, and see what you'll think about it?

I think that Google docs might work, but I don’t understand it very well, so I am happy to mail the forms to you instead! Is this your address?

The first form is an official application form, and the second is a consent form. We have everything else that we need.

I am not sure why she has started to contact me, instead of Joyce, and the first set of Doc's, I could not open, because I do not have "Word" This is the first time I was told what they really were? Keep in mind, I've been traveling for two weeks (Mom-Patrol) and then have been REALLY sick, for three? I told her to please forward to Joyce, because I am certain she has a better computer than I do, and I would trust her to sign-off for everyone. We never expected to make any money off it, right? It is just for public outreach and education? So, I think they have to go through these formalities, because they are a Non-Profit, and perhaps these quilts have to be deemed as donations. :roll:

I just sent Amanda an email asking her what this is all about and also if it would be sufficient if either you or I signed the form.
She is pretty good at getting back to me so I may hear something Monday or at least sometime during the week.
I'll let you know what I find out.

Thanks Joyce. Melissa's E-Mail ended up in my junk mail too, because I had never contacted her? Do you want her direct E-Mail Addy? I would feel more comfortable, if you signed it, since you are so involved witht the reunion, in Chicago, and I don't even know if I can go? :shock:

Just to put your minds at ease, I had to sign off on a similar form when I donated my dad's war gear to our local museum. It stated that I was releasing the items to the museum voluntarily as a gift and I could not go back and try to regain ownership of the items I had gifted.

This lady just needs in it writing that you are gifting the quilts to her association and will not come back after them. Merely a formality.

Thanks Snick. I think she is snail-mailing me the forms, and I can't imagine anyone NOT wanting the quilts, to go on tour? So, I will sign them, if Joyce doesn't beat me to it. :mrgreen:

Here is the latest E-Mail, from Melissa. I hope you all can make rhyme or reason out of it. I can't possibly track down 29 Loved Ones for this. Joyce, I hope she E-Mailed you the forms, you printed tham out, signed them, and mailed them back. I haven't received my set yet.

Hi Nancy,

The forms I sent you are the forms we give to every person who contributes a panel, and they must be signed before their panel can be displayed as part of the Quilt to Remember Tour. I have forwarded them to Joyce with a bit of an explanation on what they’re for, etc. I told her that she may sign them on behalf of the group- so long as the group is happy for her to do so. Otherwise, unfortunately, they will not be able to be displayed on the tour. If anybody has responded to your post on the forum, you may send me their response for me to attach to the consent form. So, Joyce may sign it, and any comments or authorizations that you and Joyce receive can be sent to me.

Let me know if you have any other questions- I know I’m not doing a very good job of explaining this to you!

I received the concent form and will run off a copy and sign it today. It will be in the mail this morning. Nancy, if you received a copy, you may also sign the one you received and send it in also. I told Melissa she may receive two.
Anyone wishing to send in their ok, can do it on here and I'll just copy everything and send it in.

I also ask Melissa to send me a picture of the last two squares. If and when she does, I'll copy them here.

Everything will be taken care of today so no need to worry.

I think that when this all started, we all were basically giving our permission since we were told this was going to the Alzheimer's foundation to be a part of the tour.
